Staff Evaluation Form: The Museum of Food in America
This evaluation form is to be filled out by each employee’s supervisor annually, followed by a meeting to discuss the contents. The employee should fill out the one question relating to areas the employee would like to see changes to his/her job and bring that to the meeting. Two copies of the signed evaluation should be made; one to give to Museum’s President to be put into our personnel file and the other to be kept by the employee. The supervisor, if it is not the President, may wish to keep a copy for him/herself as well.
